Is it appropriate to call oneself Dr. if it's only an Honorary degree you didn't earn?

I saw a "get rich" book out there by some guy using the title "Dr" but who is only 23 and claims to be a millionaire. Seemed a bit pretentious to me, but what do the rest of you think?
